I am the guardian of the participant and enter into this agreement on behalf of the participant as well as in my personal capacity as the participant’s guardian, under the following conditions:

1. The guardian consents to the participant engaging in any and all activities, exercises, routines, programme, games and tuition conducted by Busyballers CC.
2. The guardian understands that the participation shall be at the risk of the participant and at my own risk and that Busyballers CC its agents or employees cannot be held liable for any theft, loss damage or injury to person or property and that by signing this agreement; Busyballers CC, its agents and employees are indemnified against any such theft, loss damage or injury.
3. Busyballers CC reserves the right to change the venue at which the coaching sessions are conducted, as well as the appointed time and/or dates of the sessions for whatever reason including inclement weather, public holiday or the like.
4. Although Busyballers CC shall attempt to notify guardian’s participants of alterations to scheduled coaching sessions, it is the duty of the guardian and/or participant to confirm the date, time and venue of each session and Busyballers CC shall not be financially liable for any loss sustained as such reason.
5. It is the duty of the guardian to provide Busyballers CC with details of all known illnesses, allergies, diseases suffered, special treatment or medication required. These details must be noted under the heading “Special Instructions” here below or given to Busyballers CC should they arise in the future.
6. All fees mentioned are subject to change.
